Registered Nurse Job Description

POSITION SUMMARY

Provides services to clients in accordance with the State Nurse Practice Act, Agency policy, and accepted professional standards of practice.

The Registered Nurse provides nursing services utilizing a comprehensive base of nursing theory and nursing process, and communicates/documents observations and assessments.

REPORTS TO: Clinical Manager/Administrator

QUALIFICATIONS

• Graduate of an accredited school of professional nursing. BSN, preferred.
• Current license as a Registered Nurse in the state(s) of practice.
• Minimum of one (1) year experience in an acute care setting or equivalent experience.
• Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.
• Current CPR certification.
• Strong communication and interpersonal skills.
• Working mobile phone.
• Must be comfortable using electronic communication (secure email) as well as other means.

ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY

1. Assumes responsibility and accountability for the practice of professional nursing in accordance with the State Nurse Practice Act and standards for home health nursing.

a. Performs comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
b. Completes comprehensive assessments including OASIS data set within forty-eight (48) hours of referral for speech therapy unless other arrangements are made.
c. Develops and/or follows an individualized Plan of Care that incorporates analysis of data and current scientific findings.
d. Provides skilled interventions with a focus for achieving realistic client outcomes within a specified time period.
e. Reports changes and information necessary to modify and update the care plan to reflect progress towards goals (outcomes).

2. Consistently demonstrates competency with technical nursing skills according to personal and legal scope of practice:

a. Assessment skills as applied to the client, family/support system, and environment.
b. Teaching skills according to the client’s needs and within the context of the client’s environment, value system, and physical/emotional status.
c. Medication administrations.

3. Practices nursing with respect for individual, cultural, and spiritual differences.

    Licensed Practical Nurse (LPN) Job Description

    POSITION SUMMARY

    The LPN performs skilled nursing functions in accordance with the federal, state, and local laws and within the guidelines of his/her professional organization and the Agency.

    All functions shall be performed in accordance with the established policies and practices and state Nurse Practice Act. The LPN/LVN delivers care to clients as delegated by the RN and Clinical Manager.

    REPORTS TO: Clinical Manager/Administrator

    QUALIFICATIONS

    • Current license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse in the state(s) of employment.

    ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY

    I. Provides professional nursing care as defined in the nurse practice act and under the direction of a Registered Nurse. Communicates appropriate status/condition changes to the RN or Clinical Manager per agency guidelines.
    Provides skilled nursing visits for clients as directed by the RN or Clinical Manager. Follows the care plan established by the Registered Nurse or Clinical Manager.
    Recognizes and reports changes in client condition to the Clinical Manager, Registered Nurse and physician, as directed.
    Communicates information on a timely basis, consistent with Agency policy and urgency of the situation. Communicates verbally, as needed, and through complete and accurate documentation in the clinical record.
    Scope of clinical practice includes personal care services, monitoring and reporting findings, medication administration and monitoring, client teaching, emergency intervention, and nursing services within professional scope of practice and as delegated by the Clinical Manager or registered nurse.

    II. Promotes personal safety and a safe environment for clients and coworkers

    a. Demonstrates knowledge of safety/infection control practices by compliance with policies and procedures.
    b. Recognizes and responds appropriately to potentially unsafe situations. Demonstrates safe practice in the use of equipment.
    d. Makes recommendations on improvement of safety.

      Certified Nurses Aide (CNA)
      POSITION SUMMARY

      The CNA is assigned to specific clients, and performs personal care services to patients as directed by the RN or Clinical Manager.
      REPORTS TO

      Registered Nurse/Clinical Manager/Administrator

      QUALIFICATIONS

      • Successful completion of a formal CNA certification training program and a current CNA license.
      • Must pass a written skills test and competency evaluation prior to employment
      • Have demonstrated the minimum competencies for placement on the registry (40 hours course)
      • Be at least eighteen (18) years of age.
      • Prefer a least six (6) months of work experience in a supervised setting, preferably health care facility.
      • Demonstrated ability to read, write, and follow a written Plan of Care and take direction.
      • Good verbal, written, and interpersonal skills.
      • Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.
      • Has not been convicted of a crime under the laws of this state or another jurisdiction within last 2 years
      • Have passed an examination administered by the Agency with a score of 70% or better

      I. Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist.

      b. Shampooing
      c. Skin care/nail care
      d. Oral hygiene
      e. Shaving
      f. Dressing

      3. Performs light household services essential to health care at home, including:
      a. Meal preparation/feeding
      b. Laundry
      c. Light housekeeping

      4. Reports any observed or reported changes in the client’s condition and/or needs to the Registered Nurse or Therapist. Follows reporting guidelines in the Care Plan.

      5. Documents care provided and completes any forms required for the client’s records. Completes the appropriate records to document care given and pertinent observations. Submits documentation per Agency requirements-at least weekly.

      6. Promotes personal safety and a safe environment for clients by observing infection control practices, following Agency guidelines, and reporting unsafe situations to the Registered Nurse/Clinical Manager

      7. Demonstrates safe practice in the use of equipment. Does not use equipment until orientation about its operation has been provided. Notifies supervisor of educational needs.

      8. Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.

      9. Maintains confidentiality in all aspects of the job.

      10. Attends in-service programs to meet compliance requirements. Identifies learning needs to Agency supervisor.

      11. Performs other related duties and responsibilities as deemed necessary.

        POSITION SUMMARY

        The Home Health Aide is assigned to specific clients and performs personal care services and Activities of Daily Living (ADLs) for clients as necessary. This service helps enable clients remain in the home as opposed to a facility.
