Home Health

Ambi’s Home Health Care assists patients in their homes by providing care under the order and supervision of their doctor. With our home health care services, patients can maintain much of their independence and yet still receive the quality of care they need to recover from an illness, surgery, or other major health crisis an event

Our goal at Ambi’s Home Health Care is to ensure your safety, health, and well-being as you move toward recovery, while always remaining sensitive to whatever degree of support you require. Our care team is always ready to respond quickly and with diligence to your home health care needs.

Receiving Services

Prior to administering services, our care team will conduct an in-home assessment to determine the type(s) of home health care services you need. A personalized care plan will then be designed based on the results of your assessment and with recommendations from your doctor, Your care plan is modified as your condition changes so that we can respond to whatever your situation requires All changes in your care plan must be approved by your doctor and all care plans are reviewed every 60 days. This helps ensure that your care plan will continue to serve you throughout your recovery.

Your Ambi’s Home Health Care Team

Your care home health team always includes a Registered Nurse. Other members of your team may also include a Licensed Practical Nurse (LPN), a Certified Nursing Assistant (CNA) and / or a physical, occupational, or speech therapist, depending upon your situation and your doctor’s orders. But no matter which services you are receiving, you can rest assured knowing that your entire care team is supervised by an RN who works closely with both Ambi’s Home Health Care Clinical Director and your own doctor.
